What are the problems with the current DEN?

I think this discussion is an excellent place to apply
systems thinking.  Before we proceed to suggesting
changes, we need to look at the entire DEN system
and determine what we intend to improve.

Currently I am seeing two suggested improvements.
One, elimination of the moderator, with the rationale
being that 100% inspection is "bad," therefore it is
unnecessary.  Two, going to a web based configuration,
with no supporting justification.

Before we start focussing on the solutions we wish
to implement, I feel we need to identify the problems
we wish to correct.  I feel this is a necessary 
component of the Plan stage of the PDSA cycle.
Only after identifying the issues we wish to correct
are we able to identify the metrics to compare and
evaluate potential solutions.

So what are the problems with the current DEN
system that merit attention?
