I think a primary question is - if a manager is rated as "improvement
needed", is the "improvement" that is needed related to the system that the
manager works within, or is the "improvement needed" something within the
manager?

The key is not just elimination of forced rankings, but elimination of
personnel ratings.  The fundamental concern is that a person's rating is not
a singular item solely under that person's control - it is the sum total of
the person, their interactions with other workers, with other managers, and
with the physical systems.  No one has yet come up with a way to subdivide
out the effect of these interactions.
