'Shewhart's Charts: Myths Facts and Competitors'*.

I am pretty sure this paper is still available from SPC Press, but the
topic is also covered in Don's book "Advanced Topics In Statistical
Process Control", which I regard it as essential reading for anyone
looking seriously at the practical application of statistical methods.
