The book, The Behavior-Based Safety Process, by Thomas Krause, John 
Hildley, Stanley Hodson, Van Nostrand Reinhold, New York, 1990, 
describes a systems approach to safety that I have found to be 
effective. According to the authors, "While Deming and other industrial 
statisticians have applied scientific methodology to quality, the 
authors have applied scientific methodology to safety-related behavior 
in the workplace."
