SIPOC reference

A group of us here in Manchester have been trying to trace a reference we have to the SIPOC* model as explained by someone called Elaine Torrence (the spelling may not be correct here). The model is based on Deming's customer/supplier input-process-output model. The only reference we can find to SIPOC is from Peter Scholte but he does not present it in the same way.
Does anyone know the original reference for Elaine Torrence's adaptation?
