Some good questions. I suspect, like most things, the determination 
of what is a root cause is subjective, and in fact the concept is 
pretty much broken. There is hardly ever a single root cause for 
anything.

It's kind of a holy grail...important to look for, not possible to 
find definitively upfront (one of course will know one's got it if 
one eliminates it and sees the results).
