One problem with this and similar definitions is that it is 
completely dependent on one's definition of the system and the "size" 
of it. You may find a root cause within the system, but when you 
expand the system view, recognizing no system is not part of another 
system, then you find other root causes.

The larger the system, the larger and more unruly the root causes. 
So, while a root cause probably needs to be assessed with respect to 
the system definitions, that means the root cause is only one of a 
number of important causes, some within the system being looked at 
and some in larger systems.
