Apologies - I cant keep up with the vagaries of the internet.  The link is now http://www.chi.nhs.uk/eng/cgr/ambulance/index.shtml

Then scroll down to "Replicating the CHI analysis etc and you will find a word document entitles amb_methodology.doc

If you still have problems let me know and I will send you the file - but I cant attach it to this post.
