This one should be fairly easy.  Have the student date his/her signature
on the form, and use that as the starting time.  When the form is marked
as accept or reject by your staff, make sure they date that.  Then
capture the two dates in a database, and you have your data.

This is a little harder, but on the application form you could ask the
student "How did you hear about us?"
