Deming Model - what is it, how do you use it? purpose? any instructions?

DENizens, Can you help Judy?

I was in a meeting that discussed the use of a "Deming Model".  Not sure what the purpose of it is.  How do you use the information collected.  Also there was  much confusion over how to fill out the various sections (Suppliers, Inputs, Entrance criteria???, Customers, Ouput, and Exit Criteria?  Is there any place where I can find definitions or instructions on how to use this?

Any help you can provide is greatly appreciated.
