Considering that either the aim, theory or system elements are managed. 
people have to be educated, and trained which takes us back to The 14 
Principles, so isn't it rather easy to see why a diagnosis of an 85% rate is 
not hard since the system is open to so many variations compared to simply 
people (one element) being the problem.
