Hi

There is some literature on the system being the cause of errors.
Jim Reason calls the causes "latent pathogens"  His book is "Human
Error" (1990) Cambridge University Press.

David Wood at Ohio State claims that the system, not the individual is
the cause of all errors.  He asserts decision errors do not exist.
